Understanding Shoulder Pain: Symptoms & Causes

Shoulder pain hinders your everyday activities and impacts your overall well-being. Understanding the causes and symptoms of your shoulder pain is crucial. This knowledge helps you get the right treatment, which in turn leads to effective recovery.

Symptoms

  • Pain and discomfort: You might feel constant or occasional pain that gets worse when you move your shoulder.
  • Stiffness: It might be difficult to flex your shoulder, especially when reaching behind or overhead.
  • Weakness: Your shoulder and arm will feel weak, making it difficult to lift or carry things.
  • Swelling and tenderness: There can be swelling, and you may feel tenderness when touching the area around your shoulder joint.
  • Limited range of motion: Moving your shoulder fully, such as raising your arm, gets difficult.
  • Clicking or popping sensations: You might hear or feel clicking or popping when you move your shoulder, which can indicate joint or tendon problems.

Common causes of shoulder pain

Injuries

  1. Rotator cuff injuries
The rotator cuff comprises tendons and muscles. It helps to strengthen and stabilize your shoulder. Injuries can occur from repetitive movements or overuse. Some of the injuries to the rotator cuff include tears, tendinitis and impingement syndrome
  2. SLAP tear
This is a type of labral rupture in the shoulder. It has a ring of cartilage that surrounds your shoulder socket. This injury often occurs from repetitive overhead activities and trauma.
  3. Deltoid muscle strain
The deltoid muscle covers the shoulder. This also allows you to lift your arm. Strains in this muscle can occur from heavy lifting and sudden movements or overuse
  4. Shoulder subluxation
An unstable shoulder joint that partially slides out of position. This can be caused by trauma, repetitive strain and ligament laxity.
  5. Frozen shoulder (adhesive capsulitis)
You might feel pain or stiffness caused due to injury or from not moving the shoulder for a long period.
  6. Shoulder dislocation
Shoulder joint dislocates due to sudden injury or trauma. You will feel severe pain and loss of balance in the shoulder.
  7. Winged scapula
A winged scapula happens when the shoulder blade sticks out rather than lying flat against the back. This condition can be due to nerve damage, muscle weakness or injury.

Conditions

  1. Arthritis

Rheumatoid arthritis and osteoarthritis can cause swelling and discomfort. You will face difficulty in moving your shoulder properly.

  2. Bursitis

The bursa, a sac filled with fluid, reduces friction in the shoulder. If it gets inflamed from repeated movements, it causes pain.

  3. Tendonitis

Tendonitis means the tendons around your shoulder joint are swollen and sore. This can happen from using your shoulder too much or just from getting older.

  4. Calcific tendonitis

In calcific tendonitis, calcium accumulates in the rotator cuff tendons. It’s painful and makes it hard to move your shoulder.

  5. Thoracic outlet syndrome

This condition takes place when nerves or blood vessels between the first rib and collarbone are compressed. It causes numbness in the fingers. It also leads to discomfort in the shoulder and neck.

  6. Heart conditions

Some cardiac conditions, including a heart attack or angina, can cause referred pain to the left shoulder.

Shoulder Pain Physiotherapy: Techniques and exercises

Shoulder pain physiotherapy involves various techniques and exercises aimed at relieving discomfort and improving mobility in the shoulder joint:

Techniques

Heat and cold therapy

  • Heat therapy is applying heat with a heating pad or warm cloth to the hurt shoulder area, and cold therapy uses an ice pack or cold compress.
  • Outcome: Heat therapy helps blood flow better, relaxes tight muscles, and reduces discomfort. Meanwhile, cold therapy reduces inflammation, numbs pain, contracts blood vessels, and reduces swelling.

Ultrasound therapy

  • High-frequency sound waves penetrate deep into the shoulder tissues to provide relief.
  • Outcome: This increases blood flow and reduces stiffness and shoulder discomfort.

TENS (Transcutaneous Electrical Nerve Stimulation)

  • Electrodes are placed on the shoulder, which sends low electric currents into the skin.
  • Outcome: It reduces shoulder discomfort by stopping pain signals from reaching the brain.

Massage therapy

  • Manually manipulating the muscles and soft tissues in the shoulder region.
  • Outcome: Enhances blood flow, relaxes tense muscles, and eases shoulder pain and stiffness.

Joint mobilization

  • Gentle movements of the shoulder joint through its range of motion.
  • Outcome: You will have better joint flexibility. This also reduces pain and stiffness.

Shockwave therapy

  • Uses acoustic waves to deliver high-energy pulses to the shoulder area.
  • Outcome: Stimulates blood flow, promotes tissue healing, breaks down calcium deposits, reduces pain, and enhances mobility.

Laser therapy

  • Involves low-level lasers penetrating the skin to stimulate cellular activity in the shoulder area.
  • Outcome: Reduces inflammation, promotes tissue repair, alleviates pain, and is effective for chronic shoulder pain.

Dry needling

  • Ultra-thin needles are inserted into trigger points of shoulder muscles.
  • Outcome: Releases tight muscle bands, reduces pain, and improves range of motion, mainly focusing on muscle knots and pain relief.

Electrical stimulation therapy

  • Electrical currents are used to make the shoulder muscles contract.
  • Outcome: Improves overall shoulder function by making muscles strong and reduces muscle spasms.

Exercises

Pendulum stretch

  • How to do it: Lean over a table or chair and allow your arm to hang down toward the floor. Gently swing your arm in small circles.
  • Benefits: Helps to stretch and relax the shoulder muscles.

Wall crawl

  • How to do it: Face the wall and place your fingertips at shoulder height. Crawl your fingertips up the wall as high as possible, then slowly move them back down.
  • Benefits: Enhances shoulder stability and strengthens the muscles around the shoulder joint.

Doorway stretch

  • How to do it: Stand in a doorway and place your hands on either side of the door frame at shoulder height. When your chest and shoulders start to extend, gently lean forward.
  • Benefits: Improves the range of motion and flexibility of the shoulder joint.

Cross-body arm stretch

  • How to do it: With one arm extended across your body, bring it gently closer to your chest with the other hand. Switch arms after holding for 15 to 30 seconds.
  • Benefits: Releases tension and increases flexibility in the shoulder and upper back muscles.
