Understanding Hip Pain: Symptoms & Causes

Symptoms

  • Pain in the hip or groin: You might feel pain in your hip, groin, or buttocks regularly or from time to time.
  • Stiffness: Less flexibility and mobility in the hip area. Often worse in the morning or after periods of inactivity.
  • Swelling: Inflammation or swelling around the hip joint causes tenderness and warmth to the touch.
  • Limping: You may have limps or difficulty while walking. This might be due to pain or instability in the hip area. 
  • Clicking or popping sensations: During hip movement, you can hear or sense clicking, popping, or snapping in the hip joint. 
  • Weakness: Weakness in the hip muscles makes it difficult to stand from a seated position, carry weight and perform other physical activities.
  • Radiating pain: Hip pain that travels to the thigh, buttock, or lower back can often be accompanied by numbness or tingling sensations. 

Common Causes of Hip Pain

Injuries

It is the injury in the femur’s upper portion. It usually happens because of falls, trauma, and weakened bones due to osteoporosis. You may feel severe pain and cannot move the affected leg freely.
This means the bursae (fluid-filled sacs) in the hip joint are inflamed. It usually happens because of repeated movements or injury. Symptoms include hip pain, tenderness, and swelling.
When the femur head is displaced from the hip socket due to car accidents or sports injuries, symptoms include intense pain, inability to move the leg, and visible deformity.
There will be tears in the cartilage ring (labrum) surrounding the hip joint socket’s outer rim. These tears happen because of injury, repeated movements, or unusual bone structure. They cause pain in the hip or groin, a clicking or locking feeling, stiffness, and limited movement.
A hip contusion is a bruise on the hip area. It occurs when you take a hit or fall directly on your hip, bruising the muscles or bone.
Hip strains are caused by sudden movements and lifting heavy objects. They are the tears in the muscles and tendons around the hip. Symptoms include pain, swelling, muscle spasms, and weakness in the hip.
Tendonitis is an inflammation of the hip’s tendons caused by repeated strain or injury. Symptoms include pain, tenderness, swelling, stiffness, and difficulty moving the hip.

Conditions

  1. Osteoarthritis This condition happens when there is gradual degeneration of joint cartilage and underlying bone. Over time, joints undergo wear and tear, and natural aging is a typical cause of this ailment.
  2. Rheumatoid Arthritis Rheumatoid Arthritis causes long-term joint inflammation and movement problems because the immune system attacks the joint tissues. This leads to joint damage and pain.
  3. Avascular Necrosis Avascular Necrosis happens when there is bone tissue loss because of poor blood flow. It can be caused by injury, long-term steroid use, or heavy drinking. This condition can lead to the bone and joints breaking down.
  4. Hip Dysplasia Hip dysplasia is the abnormal hip joint development, usually evident from birth. This congenital condition leads to problems with stability and movement.
  5. Sciatica There is severe pain extending down the leg from the lower back. This happens when the sciatic nerve gets compressed or irritated. Herniated discs, piriformis syndrome, and spinal stenosis are common factors that can stress the sciatic nerve, causing pain and problems with movement.
  6. Snapping Hip Syndrome There will be a snapping sensation or sound in the hip when you move. It often occurs due to tendons or muscles sliding over bony structures in the hip, which causes discomfort and audible snapping sensations.
  7. Femoroacetabular Impingement (FAI) Here, the hip bone and socket get abnormally touched, which causes pain and limited movement. Structural issues in the hip joint cause this problem, resulting in discomfort during activities.

Hip Pain Physiotherapy: Techniques And Exercises

Techniques

1. Manual therapy

These are the techniques for moving and adjusting the joint to restore normal movement patterns. This helps to reduce stiffness and relieve pain. Massaging the muscles around the hip helps relax them and reduce tension, too.

2. Dry needling

This procedure involves injecting thin, small needles into tense muscles or trigger points. It helps reduce pain and improve mobility.

3. Electrotherapy

Methods like TENS or IFT use electrical currents to repair tissues and reduce pain and inflammation.

4. Ultrasound therapy

This improves blood flow, repair tissue, and reduces inflammation. High-frequency sound waves are used for ultrasound therapy. It can also be beneficial for bursitis or tendonitis.

5. Heat and cold therapy

Applying heat can relax tight muscles and increase blood flow. While cold therapy (ice packs) can reduce inflammation and numb sharp pain. Depending on the stage of injury or pain, these are often used in combination.

Exercises

1. Hip bridges

  • How to do it: First, lie on your back with both feet flat on the ground. Then, make your hips wide apart and your knees bent and tense your abdominal muscles. After that, lift your hips toward the ceiling so that your shoulders and knees form a straight line. Hold for a few seconds, and then slowly lower your hips to the earlier position. 
  • Benefits: This exercise improves hip joint stability and makes your glutes, hamstrings, and lower back muscles strong.

2. Clamshells

  • How to do it: Try to place your feet together and then bend your knees 90 degrees. Do this while lying on your side. Raise your upper leg as high as possible, and don’t shift your pelvis. And keep maintaining contact with your feet. After a little moment of holding, gradually bring your knee back down.
  • Benefits: Start doing this to strengthen your hip abductors and external rotators. This can also improve hip stability and reduce pain.

3. Hip flexor stretch

  • How to do it: Make a 90-degree angle with both legs by kneeling on one knee and placing the other foot in front. Up till you feel a stretch at the front of your hip, softly push your hips forward while maintaining a straight back. Switch sides after 20 to 30 seconds of holding.
  • Benefits: Do this to stretch your hip flexors. Tight hip flexors lead to pain, especially in individuals who sit for long periods.

4. Standing hip abduction

  • How to do it: Stand upright, holding onto a chair or wall for balance. Lift one leg out to the side, keeping it straight and your toes pointing forward. Lower it back down slowly and repeat. Switch legs after completing the set.
  • Benefits: Doing this exercise daily strengthens your hip abductors. This also improves your hip stability and reduces the chances of hip pain.
